Remote Care Between Visits

How the Program Works

If your provider determines you are eligible, you may be invited to participate in the Nsight Health program. Once enrolled:

1.

You may receive a complimentary medical device to measure vital signs from home

2.

You will be asked to take ongoing readings

3.

Nsight Health reviews your data for any changes or concerns

4.

You receive a brief monthly call from your Nsight Care Coordinator

5.

If any of your readings appear concerning, Nsight Health communicates back to
your provider

6.

Your care remains directed by your physician at all times

Participation is voluntary, and you may opt out at any time. Most readings take under a minute, and monthly calls usually last about 15 minutes.

What Is Remote Patient Monitoring (RPM)?

Remote Patient Monitoring (RPM) allows your healthcare provider to monitor certain health readings between visits using simple, at-home medical devices. Depending on your condition, this may include:

Blood pressure monitoring

Blood sugar (glucose) monitoring

Oxygen level monitoring

Weight monitoring (when clinically appropriate)

Your readings are securely transmitted, reviewed by Nsight Health’s clinical team, and shared with your provider as part of your ongoing care plan.

Who Is Nsight Health?

Nsight Health is a remote care services organization that supports healthcare providers across the United States by delivering Remote Patient Monitoring (RPM), Chronic Care Management (CCM), Principal Care Management (PCM), and Behavioral Health Integration (BHI) programs.

Timarron Family Medicine has partnered with Nsight Health to help extend care beyond the clinic
through:

  • At-home health monitoring, connected directly to our practice
  • Regular check-ins with a clinical care team
  • Ongoing 24/7 care support from a U.S.-based clinical team

Nsight Health works directly with patients on behalf of our practice and communicates only after provider authorization and in accordance with applicable privacy and consent requirements.

Insurance Coverage & Eligibility

Many remote care services are covered by Medicare and other insurance plans; however:

  • Eligibility depends on medical criteria and provider authorization
  • Coverage and any cost-sharing vary by insurance plan
  • Benefits will be reviewed during enrollment before participation

Participation in remote care programs is subject to provider authorization, depends on eligibility and insurance coverage, and does not guarantee specific health outcomes.

Nsight Health operates in compliance with applicable federal and state laws, including HIPAA and patient privacy regulations. All patient communications occur only after appropriate authorization and consent.
